Student Alert Form
IMPORTANT: Student Alert is designed to be used for students who can
still successfully pass a course by changing their attendance and study
habits. Students who are too far behind or have stopped/never attended
should not be sent a Student Alert, but should be sent a letter from
you or your division informing them of their need to officially withdraw.
When submitting a Student Alert, please double check the spelling of
the student's name, student ID number, and CRN. Thank you for your
help. Your student alert will be processed as quickly as possible.
